About Us | Who we serve
Grace Haven is designed for seniors who have chronic health problems or who have difficulty meeting their own social needs due to isolation or significant memory loss. The program is then modified as needed to meet the need of each individual in attendance.
Standard:
All clients admitted to the program must meet the admission criteria
Admission Criteria:
Individuals:
Who are socially isolated;
Whose physical, mental, social, and or emotional functioning is deteriorating and may cause early facility placement (Long Term Care)
Who require restorative or rehabilitive services in order to achieve optimal level of functioning
Whose care needs cannot be met in a large group setting;
Whose behaviour does not diminish other’s enjoyment of the day;
Who are capable in some form of participation in activities;
Who appear capable of attending other community programs but due to extenuating circumstances require further support (severe isolation (withdrawn from community life and whose relationships have decreased over time), unique physical or mental needs (behaviour challenges that require extra support to function in a group).
Referrals:
Referrals and requests for intake assessment may come from any source (family, friend, individual, service professional, clergy, etc.). Eligibility is determined by the Grace Haven Program Manager.
Service boundaries:
Grace Haven clients are welcome from anywhere in the region if transportation can be arranged. Grace Haven has a satellite program in Patricia Gardens.
