About Us | Who we serve
The program is designed for seniors who have chronic health problems or who have difficulty meeting their own social needs.
Standard:
All clients admitted to the centre must meet admission criteria.
Admission Criteria:
Individuals:
who are socially isolated;
who without service would deteriorate in physical, mental, social, and or emotional functioning and may require facility placement;
who because of physical, mental, and or emotional functioning are not suitable for attendance at other community programs;
who require restorative or rehabilitative services in order to achieve optimal level of functioning;
whose care needs can be met in a group setting;
whose behaviour does not diminish other’s enjoyment of the day;
who possess the necessary stamina to manage a part or full day of activities;
who are capable in some form of participation in activities;
who appear capable of attending other community programs but due to extenuating circumstances such as the following, require further support-severe isolation (withdrawn from community life and whose relationships have decreased over time), unique physical or mental needs (behaviour challenges that require extra support to function in a group).
Referrals:
Referrals can come from any source, but eligibility must be determined through CCAC and the Grace Haven manager.
Membership boundaries:
Grace Haven primarily serves the Dryden city, but does expand its service area if transportation can be arranged. Grace Haven also facilitates satellite and/or mobile programs when the interest and a need is present (e.g. Vermilion Bay, Eagle River, etc.).
